Usually, sperm quantity, motility or morphology are the main factors that are measured when determining fertility in men, but there are other aspects that must be taken into account during this analysis. One of them is sperm DNA fragmentation, a condition that can jeopardise a man's reproductive capacity if not detected and monitored.
It occurs when there is breakage or damage to the DNA strands of the spermatozoa, which can be a problem for the development of the embryos and during implantation and gestation. The damage can be mild or severe, can affect one or both DNA strands and, most importantly, is not detectable through a conventional semen analysis. That is, a spermatozoon may appear normal under the microscope, but be genetically damaged.
There are different reasons for this DNA breakage to occur, but it is often directly related to a person's lifestyle. Oxidative stress is one of the most common causes, and occurs when there is an imbalance between free radicals and antioxidants in the body. In this sense, smoking and excessive alcohol consumption, or simply old age, are other factors that can cause this fragmentation.
But there are also physical causes, such as the presence of varicocele (a dilation of the testicular veins that interferes with the correct oxygenation of the testicles), some reproductive tract infections, prolonged fever or exposure to environmental pollutants, such as pesticides or heavy metals.

When sperm DNA fragmentation occurs, the chances of fertilisation decrease, as the egg may not be able to repair this genetic damage. It can also affect embryonic development, decreasing the likelihood of reaching blastocyst stage. Even so, if there is an embryo to transfer, there is an increased risk of miscarriage, especially during the first weeks of gestation.
It should also be taken into account that embryos originating from spermatozoa with a high level of fragmentation present a lower viability, even in treatments such as in vitro fertilisation. For this reason, this type of analysis is increasingly being incorporated into the study of couples with reproductive problems.

These studies are painless and are usually performed on a semen sample, so there are no complications for the patient.
If diagnosed correctly, a number of recommendations can be made to help reduce sperm DNA fragmentation. These include adopting healthy habits such as quitting smoking, reducing alcohol consumption or avoiding exposure to excessive heat (such as saunas or laptops on the lap). Good stress management also plays an important role in these cases.

For more complex cases or when there is a rush to achieve pregnancy, assisted reproductive techniques can be used to select sperm with less damage, such as Zymot (fluid technology that allows sperm to be separated) or PICSI (selection based on hyaluronic acid binding).
It is especially indicated in men with infertility of unknown cause, in couples who have had several failed in vitro fertilisation treatments, or in cases of recurrent miscarriages with no apparent cause.
Performing this test can make the difference between continuing to try without success or finding a clear and personalised solution.
