Human Papillomavirus, commonly known as HPV by its initials, is a frequently diagnosed sexually transmitted infection worldwide. It is its prevalence—and the surprising taboo surrounding it—that drives us, the professionals at IVF-Life, to discuss its definition, detection, prevention, and of course, its relationship with fertility and assisted reproduction treatments.
Human Papillomavirus (HPV) is a primarily sexually transmitted infection. According to experts, will affect over 80% of the sexually active population at least once in their lifetime.
Currently, between 150 and 200 strains of HPV are known, about 40 of which are transmitted sexually, potentially causing skin (warts on skin and genitals) and mucosal conditions (also in the genitals or oropharyngeal region).
These strains are classified as low or high grade, based on their oncogenic risk, meaning their ability to cause cancer. While this possibility should not be alarming, as few HPV types lead to such diseases, it is necessary for society to be aware of the available options to prevent both the initial infection and its potential consequences.
One characteristic of HPV is that it doesn't always show symptoms. Some of the strains mentioned earlier often manifest through genital warts, associated with low-grade strains, with the most known being types 6 and 11. On the other hand, high-risk strains are linked to the development of precancerous intraepithelial lesions in their early stages or cancerous lesions in more advanced stages, as the majority of cervical cancer cases are related to these strains.
Since these abnormalities are not visible, the only way to detect them is through a cytology test, which can also identify the specific HPV strain involved by analyzing cervical cells.
The Pap smear or cytology is undoubtedly one of the most powerful tools women have for detecting and controlling HPV. For men, the diagnosis tends to be more complicated due to the lack of consensus on the most effective and reliable method. Men usually become aware of their condition upon noticing warts or other lesions on their bodies, being significant vectors of transmission in all cases.

It is important to understand that HPV has no cure per se, though the good news is that it often disappears spontaneously within about two years. When the infection appears or persists, regular gynecological check-ups will enable the patient and their specialist to manage the condition. The frequency of these check-ups varies greatly by country and, for instance, in Spain, it also varies by autonomous community. During these consultations, a cytology is always performed, which may be accompanied by a colposcopy.
This test allows the gynecologist to detect suspicious areas on the cervix using a colposcope — a microscope that provides significant magnification and light—since these areas change color after applying an acetic acid solution. Colposcopy is minimally invasive and not painful, though sometimes slightly uncomfortable, lasting about 10 minutes. If potentially abnormal areas are observed, the specialist can perform a biopsy, which will be analyzed in a laboratory to rule out malignant lesions.
What can be treated, for example, are some warts, either through removal or topical medication. Recently, it has also become common to encounter professionals recommending certain dietary supplements that can support the immune system, crucial for combating HPV.
Due to its prevalence among young people, numerous scientific studies have investigated the implications of HPV on fertility.
For women, the data is contradictory. While it seems that HPV does not affect women's fertility, some studies link the virus and the treatment of its lesions with an increased risk of infertility and/or complications during pregnancy and childbirth, partly due to the concomitance of HPV with other STIs and the consequent weakening of the immune system.
Women diagnosed with HPV and seeking reproductive advice can undergo fertility treatment without issue. However, if a lesion is present, it is recommended to postpone treatment until it resolves.
When this premise has been studied in men, the conclusions have been clearer, showing that an HPV infection can influence sperm quality, genetic composition, and even cause azoospermia. All these scenarios can be addressed and resolved by an experienced team like IVF-Life.
The best way to avoid HPV infection is by using barrier contraceptive methods (such as condoms) during sexual intercourse. The challenge is that sometimes these do not cover areas where lesions may exist, and therefore, infection still occurs.
This situation highlights the importance of HPV vaccination, which is currently available in Spain under various names.

Currently, in Spain, the public health system covers the vaccination for pre-adolescent and adolescent girls who have not had sexual intercourse. Some specialists also recommend its administration in men, even though it is not included in their vaccination schedule.
