Sperm, also known as semen, is the fluid produced by the male reproductive system that contains spermatozoa, the male reproductive cells necessary for fertilizing the egg. In addition to sperm cells, semen includes secretions from glands such as the prostate and seminal vesicles. These components provide nutrients, protection, and an optimal environment that supports sperm survival and motility within the female reproductive tract.
Sperm plays a key role in male fertility, as its function is to transport sperm cells to the egg and facilitate fertilization. Therefore, sperm quality is a crucial factor in the chances of achieving pregnancy.
When we talk about fertility, we must not forget that it is not something exclusive to women. The man’s contribution is essential for the fertilisation of the eggs. Without sperm, conception would be impossible.
According to the Sociedad Española de Fertilidad (SEF), sperm quality in Spain has declined significantly in recent years. The average sperm concentration has dropped from approximately 113 million per millilitre in 2001 to 66 million per millilitre in 2021. It is estimated that 50% of infertility in couples is caused by the male factor. This decline has been attributed to various factors, including lifestyle changes, exposure to environmental pollutants, and dietary influences.

Sperm quality is evaluated through a semen analysis, a diagnostic test that assesses key parameters related to male fertility. This test examines several important factors, including sperm concentration (the number of sperm per milliliter), motility (the ability of sperm to move effectively), and morphology (the shape and structure of sperm cells). It may also analyze semen volume and other characteristics that can influence reproductive potential.
Semen analysis is considered the standard method for evaluating male fertility, as it provides essential information about the ability of sperm to reach and fertilize the egg. If there are concerns about sperm quality or difficulties in achieving pregnancy, this test is usually the first step in identifying possible issues and determining the most appropriate treatment.

But, can semen quality improve? At IVF-Life we have compiled some tips that could help us maintain semen quality:

Each case is unique, but there are several factors that have been shown to reduce semen quality in men. Apart from a bad lifestyle or an inadequate diet, there are other circumstances that can contribute to this, such as exposure to toxins, stress or excessive heat in the pelvic area.
In other cases, it may be the result of medical problems, such as certain infections, hormonal imbalances or genetic problems. The exposure to treatments such as chemotherapy or pelvic radiotherapy can seriously damage sperm production. In these cases, sperm cryopreservation prior to treatment is recommended, thus keeping the most viable sperm frozen so that they can be used in a future assisted reproduction treatment.
The fact that the quantity or quality of the sperm is not optimal does not mean that paternity cannot be achieved. In our assisted reproduction clinics, we can select the sperm with aim of using those with the highest quality and, therefore, the best chances of success when it comes to the fertilisation of the egg.
In addition, testicular sperm aspiration can be performed to retrieve sperm directly from the testes by testicular puncture under local anaesthesia.
These techniques allow us to use these spermatozoa for assisted reproduction processes such as artificial insemination (AI), in vitro fertilisation (IVF) or intracytoplasmic sperm injection (ICSI).
